Responsibilities:

• Coordination of nursing teams and healthcare professionals in a home care environment.

• Planning and supervision of nursing routes and schedules.

• Clinical follow-up of patients in coordination with the medical and healthcare team.

• Management of clinical and operational incidents.

• Constant communication with multidisciplinary teams.

• Supervision of care quality and compliance with protocols.

• Preparation of reports and follow-up of healthcare activity.

Essential requirements:

• Nursing degree.

• Minimum experience of 5 years in healthcare environments (hospital, home care or clinical coordination).

• Organizational capacity and team management skills.

• Strong clinical and healthcare orientation.

• Management of clinical or coordination digital tools.

• Valid work permit in Spain.

Valued requirements:

• Experience in home hospitalization.

• Previous experience in coordination of healthcare teams.

• Knowledge in management of chronic or complex patients.

• Additional training in healthcare management or care quality.

• Intermediate or advanced English.
